How do you hold it?

By the long grip, with the attachment facing forward. The bend brings the opening to the clitoris without contortions; there the rim rests loosely and encloses it without pressing. Start on the first level and go up in small steps.

Why can the attachment be removed?

So that you can clean it thoroughly. Otherwise lubricant and water collect under the white attachment. Pull it off after every use, wash it with warm water and mild soap or a toy cleaner, and let both parts dry before you put it back on.
