What does the attachment do?

It sits over the pulse-wave end and closes it into a small chamber. That turns the pressure pulse into a sucking stimulus of the kind air-wave devices produce. It pulls off at any time.

How far does the shaft bend?

Far enough for both ends to rest against you at once. The set shape stays until you change it, including while the device is running.

Do I need a hand to hold it?

Once the bend sits, usually not. That is exactly what the shape is for, leaving both hands free. During more vigorous movement a guiding hand still helps.

May it go under water?

Yes, IPX7 means submersible rather than merely splash-proof. The socket has to be dry before charging, so after rinsing it belongs out in the open for a while.
