When do I pull them out?

Whenever you like, but always slowly. The sensation comes as each single bead passes; a yank turns that into pain.

Why does the handle have to stay outside?

Because it is the part you hold. The sphincter pulls inward whatever it can grip; the handle with the motor therefore always stays outside and in your hand.

Which lubricant fits?

How is it cleaned?
