What keeps the drop shape in place?

The slim neck. Once the wider head is inside, the sphincter closes around the neck and the plug stays where it is.

Why does the base matter so much?

Because it makes sure that part of the plug always stays outside: without a flared base, an anal plug can slip all the way in. Here the base is a requirement, not an extra.

How is it best inserted?
