How do gag and clamps work together?

Because the chains hang from the gag, every movement of the head pulls on the clamps. If the gagged person raises the chin, the pull gets stronger; if they lower it, it eases. That way every movement becomes part of the game, and that is why you start slowly.

How long may the clamps stay on?

Only briefly, about ten to fifteen minutes at a time. Turn the adjusting screw loose at first and increase the pressure slowly. Taking them off can hurt more than wearing them, because the blood flows back; so remove them gently.
