Is it worn under or over clothing?

Both are intended. Over a top it is jewellery, on the skin it is something else - the same chain, two effects.

How tightly is the collar put on?

So that two fingers fit underneath. A collar leads, it does not choke - anything tighter has no business at the neck.

Can the chain be taken off?

Through the clip. That leaves an ordinary collar behind, usable on its own.
