What is the U shape meant for?

For two spots at once. The shafts lie side by side, so one can be inserted vaginally and one anally. You can just as well use only one shaft and let the other rest against the outside.

How deep may it go?

Only so deep that the bend stays outside. It does not hold on its own, so keep it in your hand by the bend for as long as it is inside the body. Anally you go slowly and stop at once if it hurts. Toys with a wide base for the bottom are under anal plugs.

How does it stay hygienic?

By never moving a shaft unwashed from the back to the front. Bacteria from the bowel have no business in the vagina. Remember which shaft was used anally and clean it before you swap sides, or pull a separate condom over each one.
