How secure is it without a harness?

This design does not promise a firm hold: the anchor end stays in place only through the muscles of the person wearing it, and there is no retaining base. When things get stronger, a hand helps out or you switch to a harness. Neither end is ever inserted completely, and anal use is only with a hand holding the free end firmly. Why are the ends different lengths?

Because they have different jobs: hold on the body on one side, reach to the outside on the other. A matched pair would do both jobs worse, with one end too long inside and the other too short outside.

Does it need lubricant on both sides?

Yes, inside and out. At the anchor end it keeps anything from tugging, outside for the rest. Use water-based lubricant, see water-based lubricant.

How do I clean it?

Along the whole length with warm water and toy cleaner, then dried completely and stored on its own. Because both ends have body contact, they are always cleaned together, even if only one was in use. If the dildo moves between people, a condom goes over it.
