How do I adjust the fit?

With the two bands. The band at the nape decides how high the lace sits at the front; tie the cross band just tight enough for the front to lie close without cutting in. It is easiest to tie the nape first and then the back.

Can I wear a bra underneath?

Better not. A bra fastening would sit in the middle of the bare back, and at the front a bra shows through the lace. Below, black briefs work well; through the heart on the skirt they do remain visible.
