Which dildos fit?

Those with a base wider than the O-ring. The shaft is pushed through the ring from behind, the base catches behind it and holds the dildo. Without a pronounced base it slips through.

What does the velvet lining do?

Velvet lies softly on the skin where the edge of a faux leather strap would otherwise rub. You notice it on hips and thighs as soon as the harness is worn for longer.
