How do you hold it?

By the gold-coloured end, much like a pen. Your thumb then rests on the button, and small movements of the wrist guide the angled tip exactly to where it feels good.

Do you need lubricant for it?

Yes, a drop between tip and skin. Without lubricant the head rubs, and friction in that place becomes unpleasant rather than pleasant after a short while. Which setting do you start on?
