How is the length set?

The bar is extended and locked - before putting it on, not afterwards. Anyone cuffing first and adjusting later pulls on joints that do not follow.

Only for the ankles?

That is what it is built for. It works on the wrists too, but the cuffs are sized for ankles - suitable things for up top stand under BDSM sex toys.

Why not use it alone?
