How do you use the buttons?

One button is for the pressure waves, the other for the vibration. Hold it for three seconds to switch the function on or off; a short click changes the level. You can run both together or just one of them.

Why do you sometimes feel hardly anything?

Usually the opening is not sealing. It has to rest on the skin all round for the pressure waves to arrive; a drop of lubricant on the rim helps more than a higher level.
