What keeps the plug outside without a base?

The hose, and your hand on it. The sphincter pulls inward whatever it can grip, and the rectum has no natural limit. So never let go of the hose and only insert the bulb so far that its neck stays outside.

Which lubricant fits?

A water-based one, and plenty of it, because nothing gets wet by itself anally. Silicone gel does not suit silicone, it attacks the surface. The selection is under water-based lubricant, thicker ones under anal and fisting lubricant.

How do I start?

With time, in a relaxed position and with no air in the plug. Insert it deflated and only then pump, one or two squeezes at a time with a pause in between. If it hurts, let the air out straight away rather than push through.
