Can you feel the ball?

Yes, that is the whole point. It works when you move; if you lie still, you mainly feel the plug filling you. Many people therefore wear it during sex rather than instead of it.

How do I insert it?

With plenty of water-based lubricant on head and bum, slowly and without pushing if it does not go yet. Once the head is in, the neck slides into position and the base settles between the buttocks.
