Why is the anchor end so slim?

Because it is meant to sit for a long time without getting in the way. A thick anchor end tires the muscles and slips out in the end anyway. Two centimetres hold well and can be held for longer.

Can it be used classically between two people?

Yes, it works as an ordinary double dildo between two people as well, and then the holding bulb goes unused. Strapless use is the actual idea but not a condition.

How do I find the right angle?

Through the hips, not the hand. If the outer end stands too steeply, tilting the pelvis helps more than pushing. A few minutes of trying it standing up saves a lot of adjusting later.
