What are the grooves on the head for?

They give grip. On a smooth ball the skin slides along instead of the muscle beneath being worked; grooves hold against it and pass the movement inwards. For the neck and calves that is the difference between tickling and massage.

Is this really a massage device?

How is it cleaned?

Warm water and toy cleaner, keeping clear of the charging contacts at the base and letting it dry completely. The only lubricant that belongs with it is water-based lubricant, because silicone-based products dull the surface.
