What does the forked head do differently?

It grips rather than presses. Two mobile prongs lay themselves on the left and right, while a round head rests from above. At the neck and the calves that is the noticeable difference, because the muscle lies between them rather than beneath.

Is this really a massage device?

For the neck, shoulders, arms and feet yes, and the manufacturer advertises it expressly as such. A wand differs from an ordinary vibrator through its large, soft head and its power at the lower end; that suits muscles as well as everything else.

How is it cleaned?

Warm water and toy cleaner, keeping clear of the charging contacts at the base and letting it dry completely. The only lubricant that belongs with it is water-based lubricant, because silicone-based products dull the surface.
