How much does the dress show?

Quite a lot. The diamonds themselves are opaque, but skin shows everywhere through the cut-outs between them, including at the chest and back. The dress has no lining.

What do you wear underneath?

If you want to show less, a plain black thong or briefs fit underneath; you will find suitable ones under lingerie for women. A bra would stay visible through the cut-outs and break up the pattern.

Is it real leather?

No. The diamonds look like glossy leather but are made of a blend of polyester, elastane and cotton. No animal components are used in it, and the rivets have a metallic shine.
