You place the cylinder over the nipple and turn the cap; a piston inside moves up and builds the vacuum step by step. The cap is ribbed on the outside so that it turns easily, and the piston sits on a threaded rod and seals with two black rings. A nipple sucker works with negative pressure rather than pressure, which is the difference from a clamp.
