How many parts are there?

The manufacturer says five. In the photos there are a handle tube, a cone and four ball heads, so six. What actually arrives the box says.

Is a hose needed?

Yes, the one on your shower. The thread sits where the shower head normally sits - complete sets stand under anal douches.

Because of the holes. A ball head with many small bores spreads the water, a cone with one hole at the tip directs it.
