How is the vacuum built up?

With the hand bulb. Every squeeze draws out a little more air and the valve holds the level. That allows small steps instead of going to the limit all at once.

Which diameter fits?

About a centimetre and a half more than your own girth calls for. Too wide a cylinder builds no usable vacuum, too narrow a one presses against the wall - both cost more than they give.
