Why silicone instead of plastic?

Because it gives way. A hard nozzle goes exactly where you push it - including when the path is not straight. Silicone follows the anatomy and takes much of the risk out. The drawback is small: it guides marginally less precisely, which barely matters in practice.

How is it connected?

It screws onto the shower hose in place of the shower head - the usual half-inch thread fitted in Germany. That is the essential difference from an enema ball: you have running warm water, but you control the pressure at the tap and not with your hand.

How warm and how strong?

Lukewarm and weak. Hot water burns the mucous membrane before you notice, and full mains pressure can cause injuries - the bowel is not built for it. Turn the tap on before you insert, check the temperature on your wrist and work with the weakest jet that does the job.
